Please provide PHYSICAL ID (e.g. NRIC, driver’s license, passport, work passes etc) at the clinic.

Points to note before booking an appointment:

1. Book appointment ONLY if you are sure to be able to turn up for the appointment. We want to minimise cancellations and “no-shows”.

2. Do not double-book or book consecutive slots. If you need to change your booking or cancel, give us a call at 62504191 or email us at enquiry@drjeremychan.com

3. Ensure that required particulars like email and contact number are entered correctly. These are strictly used for reminders and scheduling only.

4. A confirmation email will be sent after booking. Do check your email inbox, spam/trash/junk folders for the confirmation email.

5. Keep track of your appointment in your own personal calendar. A email reminder will be sent one day prior to the appointment.

6. We are open daily except for public holidays or days which the clinic has informed regarding closure.
